Care Manager: Full-time position with benefits in a career where you can make a difference!
Mon-Fri, day-shift position provides care management services to older adults (Options and CSP programs). Duties include: performing home visits to assess social, emotional, economic, health & physical needs, and living conditions of older adults; collaborating with customers, their family members, medical providers, and/or other agencies to develop an appropriate care plan to meet identified needs; and coordinating and securing services as planned. Ongoing case monitoring and periodic reassessments are performed by employee to ensure customer needs are met. Position may occasionally serve on an on-call rotation schedule.
Our ideal candidate has excellent communication skills, a desire to advocate for the needs of the elderly, and the ability to effectively collaborate with others to ensure best service. Individuals must be detail oriented with excellent multi-tasking skills. Good computer data entry skills helpful.
Minimum Experience and Training: A Bachelor’s Degree with a social welfare major; or a Bachelor's Degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social science and one year experience in public or private human services setting; or at the agency’s sole discretion, any equivalent combination of experience, training and post-secondary education including 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science.
